The
Student Health Record must be completed and returned to the department of health and wellness services prior to registering for classes. The immunization information will help us verify that students have received the required vaccinations before arriving on campus.
Indiana’s requirements and recommendations for vaccinations
for adults, people 19 years and older
- Required: Tetanus toxoid, reduced diphtheria toxoid, and acellular pertussis vaccine
Td booster every 10 years.
- Required: Measles, mumps, and rubella.
- Required: Hepatitis B
Required after birth and must have received all three doses in six month period.
Is required for those graduating after July 1, 2008.
- Required: Polio
Three doses with third dose on or after fourth birthday.
- Required: Varicella (chickenpox)
Since January 1, 2003, unless the child has had the disease; one dose before age 13 or two doses after age 13.
- Recommended: Hepatitis A
Two doses at least six months apart.
- Recommended: Meningococcal (against Neisseria Meningitis): Meningococcal conjugate vaccine.
The department of health and wellness services offers the tetanus (Td), hepatitis B, meningococcal, varicella, and annual influenza vaccinations for a fee. We can obtain and administer other vaccines upon request.
