Student Health Form
The student health form is required of all students.  It must be submitted to the department of health and wellness services before registration.

Release of Information
The department of health and wellness services staff cannot talk to a student’s parents or significant other until this form is signed and on file in the office. The student must sign it.

Refusal to Vaccinate
Any student that does not want to be vaccinated and/or has refused vaccinations in the past must complete this form and return it to the department of health and wellness services.
