Immunization Requirements

Please complete both the Student Health Immunization Form and Student Health Registration Form. Your forms will be verified by and housed with The Student Health Center and will not be shared with or released to anyone. If you have any questions regarding accessing, completing or submitting your Student Health and Immunization Record, please do not hesitate to contact The Student Health Center at (317) 955-6154.

Marian University Required and Recommended Immunizations and Screenings

Required immunizations and screenings

MMR series
Tetanus-diptheria (Tdap), received within the last 10 years
Meningococcal (MCV4) series
Meningococcal B series
Varicella series
Hepatitis B series
Polio immunization
Tuberculosis screening is required for all international students within four weeks of arrival to the United States and for students with TB symptoms or risks. Please upload your TB screening results with the attachment tab located on the bottom of the immunization form.

Recommended immunizations

Annual Influenza vaccine, beginning in OCT for the upcoming flu season
COVID 19 series
Hepatitis A series
Gardasil immunization

The Marian University Student Health Center offers the following immunizations and screenings for a fee:

Tuberculin PPD (Mantoux) test
Quantiferon Gold TB test
Titers: Hepatitis A, Hepatitis B, Hepatitis C, Varicella, MMR
Tetanus toxoid, reduced diphtheria toxoid and acellular pertussis vaccine absorbed (Tdap)
Annual influenza vaccinations
